Intake and Eligibility Determination: When a family or individual approaches the county board for services, the Intake/Eligibility Coordinator determines if the person meets the criteria to receive services.
Emergency On-call: 24 hour/ 7 Day availability (440) 478-9029. County Board staff available all day/every day to receive calls of suspected abuse and neglect as well as other emergency situations.
Service and Support Administration: Service and Support Administrators (SSA) act as the “single point of accountability” for individuals eligible for county board services.
Quality Assurance (QA): Quality Assurance monitors for safety and satisfaction of services and staff by interviewing and surveying those delivering services.
Major Unusual Incident Reporting and Investigation (MUI): Investigation of fourteen mandated areas including abuse, neglect and rights violations.
Community Services Administration: Manage services to support community integration. Assist families/consumers to utilize Family Support Services, Supported Living, and other generic community opportunities.
Behavior Support Consultation Services: The Behavior Support Consultant assists staff, families, and/or contract agencies to develop a Behavior Plan to assure health and safety of the person and others.
Waiting List Management: Establish Waiting Lists for services requested but unavailable. Inform consumers/families/guardians of Waiting List status. To learn about getting on a list or checking your place on the list(s).
Consumer Records Management: Maintain all consumer records in a confidential manner. Handle requests for information.
