Intake/Eligibility

Eligibility is determined in one of four age groups and is updated as a person ‘ages out’ of each group. Eligibility determination for children ages 0 through 2 and ages 3 through 5 is made based on the number of delays, specific diagnosis, or type of risk factors a child has. From age 6 through 15 the individual must have at least one qualifying diagnosis, which opens the door for an evaluation that assesses functioning in several life areas. From age 16 through adulthood, a qualifying diagnosis is still the key to opening the door to administration of an adult version of the evaluation that assesses limitations. The intake process, which follows eligibility determination, helps the individual access desired services. The Coordinator assists the family or individual throughout eligibility and intake to make entry to county board services as easy as possible.
