Dear Parents Guardians:

For those of you who have children currently enrolled as a Role Model Student in a Special Education Preschool Classroom or Early Intervention classroom, we will begin accepting applications for these programs for the 08/09 school year. We will also be accepting AM/PM Respite applications. For your convenience, you may visit our website at www.gcbmrdd.org. Please keep in mind that our website is fairly new and is under construction for updating information. You may want to check the website periodically for current information. At this website, you are able to retrieve an application but one can not apply online.

All applications for the Inclusion Program must be hand delivered or received by mail and post marked no later than March 14, 2008. Applications received after that date will be considered with open enrollment. Registration will be open to the public on March 17, 2008.

Guidelines for the Inclusion Program:

1. Must be age 3 to 5 ½ years by September 30, 2008 for Preschool (fully toilet trained).

2.   Must be 18 months by September 30, 2008 for the Early Intervention.

3. Your child’s birth certificate and physical form must be on file. You will be required to update health records as needed and emergency information.

4.   A picture ID of parent/guardian registering the child.

5.   Parents/guardians must provide own transportation.

6.    Registration fee of $15.00 must be submitted by above date.

APPLICATION FEE:

There is a non-refundable $15 application fee for all classes for all students (whether re-enrolling or new). This fee must accompany the registration form.

MONTHLY TUITION/FEES:

PRESCHOOL (must be age 3 – 5.5 years by Sept. 30, 2008 and not in Kindergarten and must be toilet trained)
Days: Monday – Thursday
Times: 8:45-11:30 am or 12:15-2:45 pm      Full day: 8:45 am – 2:45 pm
$90.00 per month for half-day classes.
$180.00 per month for full day class (lunch fees are separate)
Tuition payments must be paid by the first of each month.

EARLY INTERVENTION (18 months to age 3 by Dec 31)
Days: Monday /Tuesday or Wednesday/ Thursday
Times: 9:00-11:00 am   or 1:00-3:00 pm
$50.00 per month -2 days per week
Tuition must be paid by the first of each month.
